Bond Consolidation

Complete this form if you own Ontario Savings Bonds from multiple series and are receiving more than one cheque or direct deposit statement for the interest on your Bonds. As well, you may be receiving more than one T5 slip, providing you with tax information, whether you hold annual or compound interest Bonds. This can happen if different registrations were used, or if you have moved. Consolidating your holdings will be far more convenient for you and will also help us reduce the cost of administering the Ontario Savings Bonds program.
